Self-Assessment of Parent - Student - Teacher Conferences

(Designed to Help Parents Understand How To Improve Their Child's Standing)

Parent-Student-Teacher Agenda with Two 25 Extra-Credit Point Options:

Dear Parent or Guardian of _________________________________________.

The agenda for our parent-teacher conference is scheduled for 15 minutes in duration which is sufficient to cover essential topics. I offer the following agenda so that we can efficiently and positively be focused on a variety of items of business within this time period. In absence of the actual conference, it is possible for the parent and student to self-evaluate the following agenda below and e-mail the results to Dr. Data for his assessment and together, a clarification understanding can be attained. For the convenience of using a form and earning 25 extra-credit points for your child, please go to: http://www.kuzmich.com/web_conference.html and complete it and either fax it to Carmody at: 303-982-8931 or have your child give it to Dr. Data and he will return it to you with his assessment and between parent and teacher assessments, there should be insight on where academic/behavior progress can be made as well as improved self-esteem. 1. Student demonstrates spreadsheet and Internet search techniques that were most recently taught. Database techniques are optional.

2. Thorough grading evaluation for all grades since the first week of school will be reviewed for patterns of academic integrity.
